Race and racism are based in history, and how humans thought about the physical differences. Early conceptions of these differences were focused on physical adaptations across geographies around the world. This thinking evolved over time: explanations for these differences changed as human history and science evolved. The associations between disease and race grew closer over time, however the extent to which this can be explained by science is limited. In 2022, WHO’s African Region saw 233 million cases of malaria, with 580,000 deaths. 80% of those deaths are children under 5. The phase 3 trial of the new R21/Matrix-M malaria vaccine was published in The Lancet this month, and the results suggest a turning point in the fight against malaria. Eugenics is a concept closely tied to what makes us unwell, and its roots in race medicine amplifies the drivers of racial health inequities, ableism, and white supremacy. Though scientifically flawed, eugenic thinking is present throughout modern-day society and politics. Generating scientific evidence is, although essential, just the first step. For clinical practice to be driven by evidence we must ensure that the knowledge is translated into policies that guide practice and are available for all. But how? The road from research to clinics is not a linear one, nor it is the same in every setting. Trials conducted in high-income countries can offer limited support to practice-changing in other contexts. So how do we go from evidence to implementation?
